Actions required by GP Practices

We are asking each GP Practice to complete the following 4 actions

• Switch on data sharing agreement number 6235 in EMIS or TPP

• Switch on data sharing agreement number 5668 in EMIS or TPP

• Sign the Tier 0 Tier 1 Tier 2 GP cover letter (this document has been created so GPS only have to sign one document to agree to the Share2Care tier 0 and 1 agreements and the CIPHA tier 2)

• Sign the Tier 2 S2C Data Sharing Agreement Workstream_Unified Direct Care Agreement (this is the Share2Care tier 2)

Attached below are the "how-to guides" for activation in EMIS and TPP and a copy of each of the entire tiered documents and the associated DPIAs for your reference.
